To apply for membership, complete this online registration form. Fields marked with "*" must be completed. Please follow the following procedures to facilitate payment:

1. Print out completed registration BEFORE clicking the "continue" button.

2. Affix a passport-sized photo of applicant to the printed registration form (if you do not wish to upload applicant's digital photo in the option provided).

3. Affix clear photocopy of applicant's identity card (front & back) or passport to the printed registration form.

4. Place applicant's signature in the given space.

5. Send duly completed registration form together with payment to:

6. To complete your online registration, click the "submit" button. An e-mail confirmation acknowledging receipt of your application will be sent to your indicated e-mail address.

For payment by cheque, make cheque payable to "Malaysian Alliance of Corporate Directors".

For payment by credit card, complete the relevant section and place cardholder's signature in the given space.

All applications will be processed upon receipt of payment.
